HNC Needs Assessment Survey |
July 9, 2009Dear HNC Members, Hemophilia of North Carolina (HNC) is in the process of developing our programs and services for the upcoming year. In an effort to make sure that we are meeting the needs of our members, we have created a web based survey that we are asking you to complete to help evaluate our organization. Please find a link to the assessment survey below. The survey is anonymous and confidential and will take approximately 10 minutes of your time to complete. In an effort to compile the results by the end of the month, we would ask that you please complete the survey by Monday, July 20th. As an incentive to complete the survey, we are also holding a drawing for four (4) $50 gift certificates to Wal-Mart. After you have completed the survey, please email to HNC at info@hemophilia-nc.org letting us know that you have taken part in the survey process and would like your name included in the drawing (one entry per residence please). Please include your name and phone number in the email. Four names will be randomly selected and each winner will receive a gift certificate.
